1. Whether there was a recommendation for the exposed worker to be treated with hepatitis B vaccine

2.  Whether the exposed workers was given the hepatitis B vaccination or not

3. Information that clearly shows that the healthcare provider has provided the worker with ample information on the evaluation results and any threatening medical condition arising from being exposed to OPIM or blood that would need more evaluations and maybe further treatment
