Nowadays it is a common for a physician to present all the diagnosis and treatment in a presentation software.

If a patient comes for diagnostic the physician would give him a form to fill up. This form would contain general question regarding his health and his symptoms. This data is been feed into the computer. In general terms this chat is known as patients chart which has complete information about his disease.

After the treatment process starts and all the important points are mentioned in the sheet. This it is also kept as a record of the patient for future use. Some photographs are also been added for ease of understanding the problem.
