What are the three things you should say to a patient before any procedure?

1) There are some things that can go wrong, death included, and they are .
2) I think we will do Just fine with this procedure. I've done it many times and it's always been ok.
3) You've considered all of your options and still want to go forward, knowing the risks involved.

I am assuming this is a surgical type procedure.
