The first stage is the pre-conventional level. This is a state in which decision making is based on avoiding punishments. For the Heinz dilemma, a person in this stage would say “He shouldn’t steal the drug because he might get caught and be punished.” The second stage is the conventional level. This is a state in which decision making is based on following the rules. For the Heinz dilemma, a person in  “Although his wife needs the drug, he should not break the law to get it. His wife’s  justify stealing.” The third stage is. This is one’s principle of equity and justice. For the Heinz dilemma, a person in  “He should steal the drug but alert authorities he has done it. He will have to face a penalty, but he will save a human life.”

i think it's the first one because doctors aren't the ones who get rid of the old needles. most of them time it's not even doctors who give you shots, it's nurses.
