He knows that the curtain is doing the rustling, but he isn't sure what is making the curtain rustle. It makes his heart beat loud and and fills him with terrors. This all shows that he is afraid of what may be behind his chamber door.

The other options are simply not true. Knowing that it is the wind does not create suspense. He doesn't see a visitor and we don't know if he is standing next to his bedside. The only thing that creates suspense in this passage is his fear of the unknown.
